Allergy and Immunology Coding Guidelines

Allergy and immunology billing requires precise adherence to CPT and HCPCS coding standards to ensure proper reimbursement.
  • CPT Code Range: 95115–95199
These codes represent medical services related to allergen immunotherapy. Evaluation and Management (E/M) codes may be billed in conjunction if a separate, identifiable service is provided during the same encounter.
All claims must comply with the Correct Coding Initiative (CCI) Providers should always verify applicable code combinations and payer-specific guidelines before claim submission to Medicare or commercial payers.
  • CPT Code Range: 95120–95134
These codes describe complete allergy immunotherapy services, including both antigen preparation and injection administration. However, Medicare does not recognize these codes for billing purposes.

According to the CMS Medicare Claims Processing Manual (Pub. 100-04, Chapter 12, Section 200), when a physician provides only antigen preparation services, the correct reporting codes are 95144–95170. These antigen codes are single-dose codes, and the number of doses prepared must be explicitly documented and reported.
